Athletic Physician’s Clearance is PAST DUE!

All student-athletes are required to have a Physician’s Clearance form signed by their doctor and uploaded into MC Connect. Each clearance is good for one year and must be updated on a yearly basis. If students do not have a clearance uploaded into MC Connect they will not be allowed to practice or compete.

Once the form is signed by the student's doctor, the form must be uploaded to MC Connect. Please note that parents must be logged in to their own MC Connect account (not their student’s account) to upload the form.

Steps to complete the upload process:

1. Login to the parent MC Connect account.Click on the link in the yellow banner at the top of your screen.
2. Click on the green “Review” button on the right side.
3. Click “Upload” to attach the file.
4. Type in the date of the physician’s signature.
5. Click Submit
