All student-athletes must have a
Physician's Clearance form signed by a physician and uploaded into MC Connect before participating in any MC Athletic event (practice or competition). A Physician Clearance Form is valid for one calendar year. If students do not have a completed clearance uploaded into MC Connect, they will not be permitted to practice or compete. Please note: Parents must log in to their own MC Connect account (not their student's) to upload the form.
Steps to complete the upload process:
- Log in to the parent MC Connect account.
- Click on the link in the yellow banner at the top of your screen.
- Click on the green "Review" button on the right side.
- Click "Upload" to attach the file.
- Type in the date of the physician's signature.
- Click Submit.
